Fee for Services: Our fees are based on the quality of the materials we use and our experience in performing the necessary treatment. Our goal is not to let finances prevent you from benefiting from the quality of dental care you deserve.

Outside Financing Option: As a service to our patients, we are pleased to offer Care Credit, the nation's leading patient payment plan. CareCredit has several payment plans to meet every financing need, including interest free and low interest options, each with low monthly payments and no upfront costs. Patients With Insurance: We accept most insurance plans and are happy to file claims for our patients. Our goal is to maximize your insurance benefit and make the remaining balance affordable for your and your family. Please provide the following information at your initial visit for verification of insurance benefits:
subscriber birthdate and social security number
insurance carrier, billing address, phone number, and group number
- photo identification
To verify that we are a participating provider for your insurance company, please contat us. Or, you can call the member services phone number listed on the back of your insurance card. If we are out of network for your insurance company, we will still submit your insurance claim and be happy to discuss other payment options.
Cancellation Policy: If you are unable to keep your reserved appointment, please provide us with a 48 hour notice. Our practice does not accept cancellations by voicemail outside of regular business hours. Please call during our regular business hours to change or cancel any existing appointments. Thank you!
